TERMS AND CONDITIONS
Each Travel Certificate is good for one-time use by one person in accordance with the travel restrictions identified on or in connection with the Certificate. All travel must be booked and flown within the specified validity dates. In the event that a Travel Certificate does not include dates of validity, the Certificate shall be good for travel booked and flown within one (1) year of the date of issuance. If specific flights or departures/destinations are not identified on or in connection with the Travel Certificate, the Certificate may only be redeemed for one-way JetBlue flights to/from JetBlue cities. (Not all JetBlue cities connect; call 1-800-JETBLUE (538-2538) for information on connecting cities.) Redeemed travel is subject to availability, blackout dates, and capacity controls (flights to/from Caribbean destinations subject to heightened capacity controls), and seats may not be available on all flights. Flight schedules are subject to change without notice. Certificate is not valid for bookings on partner or code share or interline flights or for use in connection with any other products or services, except that Certificate may be applied towards the air portion of a JetBlue Getaways vacation package. Certificate is transferable; however, at no time may it be purchased, sold, bartered, traded on an auction website or otherwise exchanged for value without prior written consent from JetBlue. Certificate cannot be partially redeemed, is not redeemable for cash, and has no cash

Government taxes and fees apply to travel booked using a Certificate, are the responsibility of the passenger, and will vary based on route. Taxes and fees may include, without limitation, Passenger Facility Charges of up to $9 each way; September 11 Security Fees of up to $5 each way; Federal Segment Taxes of $3.80 per domestic segment; U.S. Arrival/Departure Tax of $16.70 each way for Puerto Rico and US Virgin Islands travel; U.S. government excise tax of $8.40 per departure for Alaska and Hawaii travel; government fees and taxes up to $120.80 each way for international travel; and a Value Added Tax of up to $212.28 each way for Barbados, Colombia, Mexico, Dominican Republic, and Costa Rica travel. Government taxes/fees are subject to change as required by law. Passenger must pay all applicable taxes and fees using a major credit card at the time of booking.

A Travel Certificate may be redeemed only by calling 1-800-JETBLUE (538-2583) and providing the authorization number and PIN number included on the Certificate. Travel Certificates may not be redeemed at airport ticket counters or online. The authorization number and/or PIN number cannot be replaced if lost or stolen (except as required by law), and JetBlue is not responsible for lost or stolen Travel Certificates, authorization numbers or pin numbers. Customers will not be able to redeem their Travel Certificate without both the authorization and pin numbers. Redeemed travel is not eligible for TrueBlue points. (TrueBlue is JetBlue’s loyalty program.) JetBlue’s change/cancel policies (including certain change/cancel fees) apply. If you are a no-show for your flight or do not cancel prior to scheduled departure, you will forfeit the Certificate(s) and all money associated with the reservation. OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN.  
A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING.

Participation in the Dance Theatre of Harlem’s Dark and Lovely Beauty Giveaway Sweepstakes (the “Sweepstakes”) constitutes your agreement to be bound by these Official Rules.

Eligibility:  Open only to legal residents of the United States, 18 years of age or older at the time of entry.  Employees, officers, and directors of Sponsor, its subsidiaries and its advertising and promotion agencies, and their immediate family members and persons living in their same household, are not eligible to participate in the Sweepstakes.  Void outside of the 50 United States and where prohibited by law.  
    
Entry:  You may enter the Sweepstakes:

Automatic Entry:  Customers who enter/provide code: DLO or DTHL when purchasing tickets to a Dance Theatre of Harlem N.Y. Season performance during the Sweepstakes Period will be automatically entered to win.  You will receive one entry for each ticket purchased.

Mail:  Send your name, address, daytime and evening phone numbers, and e-mail address (if you have one) in your own handwriting on a 3” by 5” index card to: Dance Theatre of Harlem’s Dark and Lovely Beauty Giveaway Sweepstakes, 466 West 152nd Street, New York NY 10031 .  Mail entries must be enclosed in a 4” by 9½” envelope with your name and return address placed on the front left-hand corner of envelope.  You may enter as many times as you like, but only one entry allowed per envelope.  

All entries become the property of Sponsor and will not be returned.
Use of any automated system to participate is prohibited and may result in disqualification.  No mechanically reproduced entries will be accepted.  

Sweepstakes Period:  The Sweepstakes begins on April 7, 2015 at 12 a.m. (midnight) Eastern Time and ends on April 11, 2015 at 5:00 p.m. Eastern Time.  All mail entries must be received by April 11, 2015.  

Prize:  Three winners will be selected.  Each winner will receive a Dark and Lovely / Spring Beauty Collection with an approximate retail value of $150.  Both prizes will be awarded.  No alternative prize, cash or other substitution is permitted except in the sole discretion of the Sponsor in the event of prize unavailability.  All federal, state and/or local taxes are the sole responsibility of the winner.  Each winner will be given a Form 1099-MISC and will be required to supply Sponsor with his or her social security number.
 
Selection of Winner/Odds of Winning:  The winners will be selected from all eligible entries received in a random drawing on or about April 27, 2015.  Winners will be notified by phone and e-mail.  If a prize notification or prize is returned as undeliverable, the prize will be forfeited and will be awarded to an alternate winner.  As a condition to receiving the prize, the winner will be required to sign and return an affidavit of eligibility and liability and publicity release (“Affidavit and Release”).  The winner must agree to the use of his or her name, voice and likeness for promotional purposes without further compensation, unless prohibited by law.  Winner should allow approximately 3 to 4 weeks after mailing the Affidavit and Release to receive the prize.  The odds of winning a prize depends on the number of eligible entries received.

General Conditions:  No purchase necessary.  A purchase will not improve your chances of winning. Sponsor reserves the right to cancel, suspend and/or modify the Sweepstakes, or any part of it, if any fraud, technical failures or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of the Sweepstakes as determined by Sponsor in its sole discretion. In such event, Sponsor reserves the right to award the prizes at random from among the eligible entries received up to the time of the impairment.  Sponsor reserves the right to disqualify, in its sole discretion, any person acting in an unsportsmanlike or disruptive manner, or tampering with the entry process, the operation of the systems required to conduct the Sweepstakes or operate the Sponsor web site, or who is otherwise in violation of these Official Rules.  In the event of a dispute as to any registration, the authorized account holder of the email address used to register will be deemed to be the registrant or player. The “authorized account holder” is the natural person assigned an email address by an Internet access provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the submitted address. Potential winners may be required to show proof of being the authorized account holder.

Limitations of Liability:  Dance Theatre of Harlem and Dark and Lovely are not responsible for and shall not be liable for: (i) misdirected or delayed mail, e-mail or text messages; (ii) telephone, telecommunications, hardware or third party software malfunctions, failures, or difficulties of any kind; (iii) any injuries, losses, or damages of any kind arising in connection with or as a result of participating in the Sweepstakes or possession or use of any prize; (iv) any incorrect or inaccurate information, whether caused by entrants or by any of the equipment or programming associated with or utilized in the Sweepstakes; (v) any printing or typographical errors in any materials associated with the Sweepstakes; or (vi) any condition caused by events beyond Sponsor’s control that may cause the Sweepstakes to be disrupted, corrupted or terminated.  No more than the stated number of prizes will be awarded.  In the event that production, technical, programming or any other reasons cause more than stated number of prizes as set forth in these Official Rules to be available and/or claimed, Sponsor reserves the right to award only the stated number of prizes by a random drawing among all legitimate, unawarded, eligible prize claims.

Disputes:  Except where prohibited, entrant agrees that: (1) any disputes, claims and causes of action arising out of or connected with this Promotion or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by a state or federal court located in New York, New York; (2) any claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Promotion, but in no event attorneys’ fees; and (3) under no circumstances will entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, indirect, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant, shall be shall be determined by Sponsor in its sole discretion and governed by the laws of the State of New York, without giving effect to any choice of law or conflict of law rules (whether of the State of New York or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of New York.
